* Facilitate timely referrals to vital services, including medical and mental health care, counseling, legal aid, housing, and education.
* Work closely with service providers to ensure seamless coordination and follow-up support for youth struggling with mental health.
* Engage in regular collaboration meetings with community stakeholders, participating in mental health collaborative, sharing information, and coordinating joint initiatives.
* Conduct outreach and education sessions to raise awareness about mental health and empower individuals to seek assistance.
* Participate in monthly court appearances along with the probation officer, therapist, program supervisor, district attorney, and defense attorney to update all on the progress of the youth.
* Provide an initial screening and assessment of needs by utilizing a screening tool(s) and will develop an initial and ongoing assessment(s) to determine an individualized resource needs approach.
* Provide evidence-based programming, mental health education / awareness, curriculum, Parent Support classes, life-skills development, vocational skills, and independent living training.
* Attend and receive training hours on mental health and other related topics.
* Provide transportation to youth and their families as needed using a county issued vehicle.
* Collect and maintain grant data.
* Subject to irregular work schedule including two evenings and Saturdays.
* Participates in activities and duties related to emergency management during a local state of disaster as directed by appropriate county managers.
** MINIMUM

JOB REQUIREMENTS:

*** Bachelor’s Degree in Social Services, Criminal Justice or other state  approved related field.
* 3 years of job related experience.
* Minimum 21 years of age.
* Good computer and organizational skills; ability to complete assigned projects in a timely manner.
* Good verbal and written communication skills; interpersonal skills and ability to deal effectively with the public, other employees, and elected officials.
* Valid Texas Driver's License.
** STARTING SALARY:** $38.44 per hour

* 1) A felony conviction against the laws of this state, another state, or the United States within the past 10 years; a deferred adjudication for a felony against the laws of this state, another state, or the United States within the past 10 years or current felony probation or parole

* 2) A jailable misdemeanor conviction against the laws of this state, another state or the United States within the past 5 years; a deferred adjudication for a jailable misdemeanor against the laws of this state, another state, or the United States within the past 5 years or current misdemeanor probation or parole

* 3) Registered as a sex offender under Chapter 62, Texas Code of Criminal Procedure.
