Required:
Yes, 4 Advanced Practice Clinicians (APCs) - Patient Age Demographic:
Adult/Geriatric - Documentation System/EMR: EPIC
- Procedures
Required:
Electrocardiograms (EKGs)
Requirements:
- Board-Certified in Advanced Heart Failure Cardiology, Ventricular Assist Device (VAD) Certified
- ACLS Certification
- AHF Fellowship with LVAD experience
- Provider Must be Local
- DEA Certification
- Active VA License
