Practice Setting:
Inpatient/Call/Outpatient/Clinic Reason for Coverage: 2nd Physician support needed during recruitment Dates of Coverage: ASAP until Physician is onboarded Call Ratio: 1:6 Average Number of Clinic Patients Per Day: 10 Average Number of Inpatient Admissions Per Physician Per Day: 12-15 Number of Cardiologists Physician in Group: 1 Will Other Cardiologists be Present During our Locum's Coverage:

Electrocardiograms (EKGs)
Estimated Credentialing Timeframe: 120 days

• Provider must be Ventricular Assist Device (VAD) Certified (this is for Advanced Heart Failure, the provider will not perform any invasive procedures)

• Provider Must be local - only mileage is reimbursed

• A Self-Query NPDB report is mandatory at the time of presentation. The Self-Query NPDB report must be dated within 30 days of the presentation date.
