Position: Clinical Pharmacist Specialist (Critical Care/Acute Care)

Summary

This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200
-000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements:

  • United States Citizenship:
    Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy
  • Education:

    Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.

    D.) degree

    Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree programs
  • today the sole degree is Doctor of Pharmacy) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification - which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEF iBT)
  • Licensure:
    Hold a full - current - and unrestricted license to practice pharmacy in a State - Territory - or Commonwealth of the United States (i.e - Puerto Rico) - or the District of Columbia
  • English Language Proficiency:
    Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d)
  • Grade Determinations:
    To be creditable - the experience must have demonstrated the use of knowledge - skills - and abilities associated with professional pharmacy practice
  • Ability to communicate orally and in writing to persuade and influence clinical and management decisions
  • Expert understanding of regulatory and quality standards for their program area
  • Ability to solve problems - coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise
  • Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy
  • Advanced skill in monitoring and assessing the outcome of drug therapies - including physical assessment and interpretation of laboratory and other diagnostic parameters
  • Meets criteria to work under a scope of practice: 1
  • Completion of a Pharmacy Practice or Specialty Residency with substantial component (>50%) of direct patient care activities involving comprehensive medication management OR 2
  • Completion of a minimum of 3 years of pharmacy practice experience with a substantial component (>50%) of direct patient care activities involving comprehensive medication management

    Preferred Experience:

    Experience in inpatient health care setting required
  • PGY1 and PGY2 in acute care or critical care preferred
  • Must be eligible for scope of practice

  • Physical Requirements:

    The incumbent must be able to perform all activities of the position without restriction
  • This position involves:
    Must be in overall good health
  • The work requires standing - walking - moderate lifting and carrying of items
  • The work also requires the use of fingers - the ability to read without strain (far vision correctable in one eye to 20/20 and 20/40 in the other) - and the ability to hear (aid permitted).
Duties
  • Duties to include but not limited to:
    Clinical Functions Coordinates all clinical pharmacy functions for his/her assigned practice area - helping to achieve positive patient centric outcomes through direct and indirect interactions with patients - providers - and interdisciplinary teams
  • Attends and participates in multidisciplinary rounds - team meetings - conferences - and other scheduled events as required
  • Designs - implements - assesses - monitors and documents therapeutic plans utilizing the most effective - least toxic - and most economical medication treatment for patients consulted to his/her care and other clinics as designated by supervisor
  • Performs initial and periodic assessment of patients' medication therapy - monitors as needed - and makes recommendations or changes (within scope of privileges) as necessary
  • Frequency and timeliness of reviews will be determined by The Joint Commission - and/or other regulatory requirements
  • Educational Responsibilities Demonstrates knowledge of the standards related to Hospital services and Acute/Critical Care unit drug use
