Job Summary

As a Trauma Complex Care Social Worker Case Manager, you will assist inpatient trauma patients in accessing the services needed for continuity of care upon leaving the hospital. You will collaborate closely with community partners and hospital teams to provide home health, hospice, skilled nursing, assisted living, private care and durable medical equipment services.

Responsibilities

• Perform thorough assessments and develop appropriate care plans for high‑risk patients with discharge planning or psychosocial needs.
• Review patient notes to identify and address conflicts in medical orders or management gaps.
• Build rapport by providing continuity of care and acting as an advocate throughout a patient’s stay.
• Facilitate communication between multiple caregivers and foster collaboration between medical teams and nursing staff.
• Participate in interdisciplinary care planning with the Trauma Team.
• Coordinate care to optimize level of care, length of stay and resource utilization.
• Identify and overcome barriers to discharge by engaging relevant personnel.
• Initiate and monitor the referral process for post‑discharge patients.
• Track and record Avoidable Days (AD) and intervene to reduce them when appropriate.
• Manage the denial and appeal process in accordance with policy.
• Maintain awareness of current health‑care trends and ensure compliance with professional, regulatory, governmental and organizational standards.

Qualifications

• Accredited graduate of a social work school.
• Master’s degree in Social Work (MSW) required.
• Licensed Clinical Social Worker preferred.
• Three or more years of social work experience in an acute care setting preferred.
• Previous case management or utilization review experience preferred.
