Primary Function/General Purpose of PositionThe Critical Care Clinical Pharmacy Specialist participates in clinical programs and initiatives >90% of the time with a consistency in schedule. The Clinical Pharmacy Specialist has received specialized training in their dedicated area of practice and ensures safe, rational, and cost-effective therapy in their respective practice area. The Clinical Pharmacy Specialist will act in compliance with all applicable rules, regulations, and standards as promulgated by Federal, State, and accrediting agencies or regulating bodies.
The Clinical Pharmacy Specialist maintains established departmental policies and procedures while promoting department objectives.
- Provide comprehensive pharmaceutical care to the critically ill population and serve as a subject matter expert in area of specialty
- Lead clinical programs to improve patient care, optimize medication therapy and promote health and disease prevention
- Perform medication reviews and medication therapy management to ensure safe and appropriate use of medications and achieve defined patient care outcomes
- Identify and promote cost-containment efforts towards both medication utilization and best use of pharmacy resources
- Maintains proficiency in order processing and supports pharmacy operations in medication distribution and operational functions to facilitate accurate and timely medication availability
- Collaborate and communicate, both verbally and in writing, with pharmacy team members, other health care professionals, and leaders to deliver high-quality pharmaceutical services and enhance departmental and organizational programs
- Perform medication education to improve patient understanding and adherence consistent with department guidance
- Provide education to pharmacy staff and serve as a preceptor for students and residents as assigned

Licensing/CertificationMaintains current license for pharmacist in the state(s) where practicing (required)
ACLS Certification (preferred)
EducationGraduate of an ACPE accredited School of Pharmacy with a BS or Pharm
D (required)
Advanced pharmacy degree – Doctor of Pharmacy, Masters, Residency (preferred)
Work ExperienceAdvanced knowledge and skills required to perform complex pharmacy functions; residency training or equivalent practice experience in associated pharmacy practice area or equivalent practice experience required to sit for Board certification in associated practice area (required)
PGY2 or 4 years of equivalent practice experience;
Board Certification in associated pharmacy practice area (Ambulatory Care, Cardiology, Critical Care, Sterile Compounding or other practice area approved by the Board of Pharmacy Specialties or other certifying board) (preferred)
Exhibit proficient use of all computer hardware and software (required)

Demonstrates knowledge of the principles of growth and development of the life span and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his or her age, specific needs and to provide the care needed as described in departmental policies and procedures.
