Primary PurposeWe are seeking a highly skilled and compassionate certified Nurse Practitioner to join our provider network. In this role, you will conduct in-home wellness risk adjustment assessments for Medicare members and other at‑risk populations, ensuring accurate and comprehensive risk adjustment coding to enhance value‑based care. This is a 1099 (PRN) position offering flexibility for those with competing priorities.
NP Responsibilities- Perform annual in‑home wellness visits and risk adjustment health assessments on a population with chronic conditions, including comprehensive assessment, physical examination, body system review, vital signs, medication history, current prescriptions, member functional status, pain management, cognitive assessment, home environment review, fall risk screening, social assessment, daily living activities, mental health screening, documentation, and communication.
- Provide individualized quality care to the elderly population in their homes.
- Deliver patient health education on topics such as pain management and medication compliance.
- Build relationships of trust with members through exceptional communication and empathy.
- Assist in closing quality care gaps such as screenings and labs.
- Collaborate enthusiastically to enhance care delivery.
- Commit to a minimum of 30 hours per month.
- Henrico County, Virginia
- Valid, unencumbered Nurse Practitioner license for the state of employment.
- Travel up to a maximum 55‑mile radius within the assigned county.
- Preference for weekday and standard working hour schedules.
- Previous in‑home risk assessment experience preferred.
- Previous 1099, PRN, part‑time experience preferred.
- Minimum 3 years of patient care experience (primary care, adult, geriatric, EMR).
- May be required to obtain additional NP licensure supported by Advantmed.
- Reliable transportation to travel to member homes.
- Strong EMR system proficiency.
- Ability to work independently.
- Bilingual language skills a plus.
- Competitive per diem compensation (~$100 per completed assessment).
