Sign-On Bonus: $7,500

As a Matrix provider, you will:

• Conduct thorough assessments, including medical history, diagnosis, treatment, and health education.
• Administer and order necessary point-of-care and lab-based testing.
• Collaborate with physicians, case managers, and caregivers to ensure comprehensive care.
• Provide services in the member’s home or Skilled Nursing Facilities.

Qualifications:

• Master’s Degree or equivalent experience with NP or PA licensure.
• Current NP/PA licensure with prescription authority.
• Board certification in relevant specialties (e.g., Adult, Family, Emergency).
• 1 year of NP or PA experience.
• Able to travel 80-90% of the time, including overnight and weekend travel.
• Strong communication skills and proficiency in medical record software.

• Work-Life Balance: Full-time providers typically work Monday to Friday, 8:00 AM to 5:30 PM.
• Competitive Pay: Annual base salary ranging from $105,500 - $168,400 .
• Incentive Program: Uncapped quarterly incentives.
