POSITION SUMMARY
Thrive Skilled Pediatric Care's (Thrive SPC) purpose is simple, to provide high quality clinical home care to medically fragile children so they can grow and flourish to their full potential. Administers skilled nursing visits to patients requiring intermittent skilled professional nursing care, nursing service and teaches the patient, family, and other members of the nursing team. Each Registered Nurse performs nursing services for patients within a specific geographic area. These services are performed in accordance with the physician's orders and the established plan of care, under the direction and supervision of the Clinical Supervisor, Visit and Therapy Operations, and / or Clinical Director, Visit and Therapy Operations.
HIRINGREQUIREMENT
Education, Skills & Experience (Essential):
Must be a Registered Nurse licensed in the state (or a compact state with the state) in which he / she practices with a knowledge of general nursing theory and practice, including those basic bodies of knowledge relevant to nursing, such as drugs and the biological, physical, social, and medical sciences, and their applications for a better understanding of patient care problems.
One-year acute care preferred; 6 months pediatric experience in an acute or home health setting is required, or extended preceptorship as required by nursing knowledge. Previous experience in home health or home care preferred. A graduate of an approved school of professional nursing.
- Must possess active and valid professional licensure in good standing in the state of employment (or a compact state).
- Must be able to work independently.
- Must possess a valid state driver's license.
- Must possess automobile liability insurance as required by law.
- Must always wear seat belts while driving.
- Dependable transportation kept in good working condition.
- Must be able to drive an automobile in all types of weather conditions.
- Conducts skilled nursing visits, care and services for pediatric patients in accordance with physician orders and plan of care.
- Utilizes the nursing process to deliver nursing care to patients.
- Provides safe, competent care for patients.
- Nursing practice and documentation is provided in accordance with Thrive SPC standards of care, policy & procedure as well as all regulatory and accrediting agency guidelines.
- Teaches and assists the patient and / or family in functioning independently in the care of their child.
- Provides direct patient care, evaluates outcomes of patient care, consults and coordinates with other disciplines as required and adjusts nursing care processes as necessary to assure optimal patient care.
- Develops the Nursing Care Plan through assessment of patient's needs. This assessment will include a complete systems review, safety assessment, nutritional evaluation, medication profile, and environmental issues. Plan of Care is developed with the Clinical Supervisor, family, and based on the physician's orders.
- Reevaluates patient's needs with respect to the physical assessment, the plan of care, and the patient's response to care and interventions.
- Communicates changes with the patient's physician.
- Always demonstrates respect for the rights of the patient and family and respects patient confidentiality of any and all information relating to the patient.
- Insures an adequate stock of supplies and proper functioning of equipment.
- Completes all documentation requirements in accordance with Thrive SPC's policy.
- Provides patient updates and patient change in status to the Physician in a timely manner for the purpose of coordination of care and to support securing ongoing authorization for continuing care.
- Acts as preceptor to new employees as needed.
- Maintains and enhances professional knowledge base by participation in staff meetings, conferences, in‑services, and professional organizations and journals.
