Licensed Practical Nurse (LPN)

Tryfacta is seeking a Licensed Practical Nurse (LPN) for our client in Richmond, VA 23224. This is a temporary contract assignment.

Temporary contract, per diem/shift-based (ongoing)

Work Schedule:

Evening 3:00 PM – 11:30 PM (8.5-hour shifts including a 30-minute unpaid meal). Weekend and holiday shifts available.

Responsibilities for this position include, but are not limited to:

• May include charge nurse responsibilities as assigned.
• Administer medications and treatments accurately for assigned residents. Consult with physicians; receive, record, and implement orders, and schedule appointments.
• Document residents' health changes through acute care nurse's notes and shift change nursing reports.
• Formulate plans of care for new residents and prepare all required reports and documentation.
• Conduct daily rounds, observing and evaluating the physical and emotional status of assigned residents and taking necessary actions to ensure their health and well-being

To be considered for this position, you should have:

• Active, valid, and unencumbered Virginia or multistate (compact) Licensed Practical Nurse license.
• Minimum of one (1) year of experience as an LPN. Nursing experience in a long-term care or adult geriatric setting preferred.
• Current CPR certification.
• Negative TB test (PPD) or chest X-ray within the last 12 months.
• Ability to pass a criminal background check.
• Reliable transportation to and from the facility.
• Ability to work the full scheduled shift.
• Ability to follow facility dress policy (facial piercings covered, fingernails no longer than 1/4 inch).
