Housecall Nurse Practitioner — In-Home Health Evaluations

CVS Health, through Signify Health, seeks full-time clinicians (NPs or PAs) to perform in-home health evaluations, engaging with members in the comfort of their own homes and helping bridge gaps in care.

You’ll travel locally and within licensed states, conduct comprehensive reviews, document findings via iPad workflow, and provide Diagnostic & Preventive Services as ordered by plans.
