The Case Manager (CM) will be responsible for resident case management within a home for pregnant women. The primary goal is to assist residents in achieving family, self-development, and independence through case management, individual counseling, and supportive services. The CM will ensure the maintenance of safe, secure, and healthy housing for expectant and parenting single mothers and their children.
The CM will develop, monitor, and evaluate treatment plans, assess treatment needs, and provide necessary documentation. Responsibilities include conducting initial interviews, intake, admission, and onboarding of new residents, as well as maintaining confidentiality and interacting with the House Manager to plan individualized care. The CM will also coordinate services by linking residents to social services such as Passport, SNAP, WIC, and 3 Cs, advocating for their needs, and monitoring progress.
This is a part-time position requiring a minimum of 20 hours per week. The ideal candidate should possess a servant spirit and a compassionate approach to supporting pregnant women.
