Boba Care Group B.V. zoekt een autismecoach / ambulant begeleider voor 28-36 uur per week in Ridderkerk. Je werkt ambulant en op afstand met een vaste groep cliënten, variërend van kinderen tot volwassenen, onder volgens de WMO/Jeugdwet, WLZ en samenwerkingsverband.
Je voert startgesprekken uit, stelt zorgplannen op en voert evaluaties uit. Je werkt nauw samen met netwerken, levert intervisie en bespreekt casussen in teamoverleggen.
