We’re looking for dedicated Case Manager Registered Nurses who are excited to provide personalized care for patients in their homes. In this role, you’ll assess patient needs, coordinate care plans, oversee multidisciplinary interventions, and ensure quality outcomes through thoughtful case management. You’ll make a meaningful impact by combining strong clinical knowledge with empathetic care.

Key Responsibilities- Perform assessments, planning, implementation, and evaluation of patient care
- Coordinate care among team disciplines and monitor intervention outcomes
- Recommend changes to care plans based on patient progress
- Provide supervisory care and leadership within scope of practice
- Ensure documentation is timely and accurate in the electronic medical record
- Support agency compliance with federal and state regulations
This role involves working closely with our team to deliver effective, patient- or mission-focused services and contribute to a supportive and collaborative work environment.
Qualifications- Graduate of an accredited school of nursing
- Current California RN license
- PHN certification and BSN preferred
- Minimum 1 year home health experience; 1 year acute care preferred
- Current BLS certification required and maintained
- Experience with central lines, mid lines, and wound vacs
- Valid CA driver’s license with excellent driving record and automobile insurance
- EMR experience;
McKesson Homecare preferred - Strong leadership and case management skills
- Knowledge of state and federal home health regulations preferred
- Ability to read, write, understand and communicate effectively in the English language
- Must be able to put into practice Service Excellence skills with our patients, families, coworkers, and community
