Job Description # We#re looking for compassionate RNs to join our Utilization Review team, supporting patients from admission through post-discharge. In this role, you#ll guide patients and families through their care journey, coordinate safe discharges, and support responsible use of resources and services. Key Responsibilities # Coordinate care across the patient#s hospital stay, including pre-admission and post-discharge# Assess medical-social needs and available resources by interviewing patients and families# Develop individualized discharge plans, ensuring timely and safe transitions# Assist in placement to facilities such as SNFs, rehab centers, or specialty hospitals# Collaborate with teams to optimize resource use and reduce length of stay# Provide social service support in partnership with MSW staff# # Collaborative Teamwork# This role involves working closely with our team to deliver patient-focused services and contribute to the overall well-being of our patients while contributing to a supportive and collaborative work environment.#
