As trusted community-based, nonprofit health agency, RVNAhealth provides high-quality care that supports patients where it matters most:  are currently seeking a skilled and compassionate Physical Therapist to join our team and provide in-home therapy services across Northern Fairfield County, including Danbury, Newtown, Bethel, New Milford and surrounding towns.

• Perform comprehensive physical therapy evaluations and collaborate with physicians, nurses, patients, and families to develop individualized care plans.
• Conduct OASIS assessments.
• Deliver goal-oriented, evidence-based therapy in the home setting to help patients regain function and independence.
• Provide education and instruction to patients, families, and caregivers.
• Identify and refer for additional services or next level of care as needed.

• Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program.
• Connecticut PT license in good standing.
• 2+ years of clinical experience (home health experience preferred).
• Proficiency in OASIS documentation and EMR systems (tablet provided).
• Strong understanding of therapy best practices in home-based settings.
• Excellent communication, time management, and problem-solving skills.
• Valid driver's license and reliable vehicle for travel throughout Fairfield County.

The pay range for this position is $90,000 - $110,000 per year and represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications.
