Position: LICENSED CLINICAL SOCIAL WORKER

Overview

Mountain View Hospital is looking for a Licensed Clinical Social Worker to join our team!

The MVH Licensed Clinical Social Worker (LCSW) supports MVH’s initiatives toward clinical integration by improving quality and patient satisfaction, improving outcomes and the overall health of the patient, and reducing the total cost of care. The LCSW will provide assessment, intervention, and advocacy for patients with varying levels of mental health conditions. This role requires a high degree of flexibility, independence, and knowledge of the health care system, with working knowledge of community resources to maximize emotional, social, and physical well-being of patients served by MVH Primary Care Practices.

The LCSW will coordinate interventions with PCPs and other care providers as indicated.

Responsibilities

Provide assessment, intervention, and advocacy for patients with mental health conditions. Coordinate intervention with PCPs and other care providers as indicated. Support MVH’s quality and patient satisfaction initiatives and assist in achieving improved health outcomes and cost of care reductions. Maintain working knowledge of community resources to maximize patient well-being in MVH Primary Care Practices.

Qualifications

Education/

Certifications:

Licensed Clinical Social Worker.

Experience: 6 months required experience; 2-5 years clinical experience preferred.

Equipment/Technology: Experience with Word, PowerPoint, Excel, Microsoft Office, and various EMR systems.

Language/Communication: Must be patient, courteous, compassionate, and respectful with patients and their families. Effective written and oral skills. Ability to comprehend and follow detailed instructions. Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, patients and their families, co-workers, physicians, and administrative staff.

Speaks clearly and persuasively in positive or negative situations. Ability to listen well and seek clarification.

Mathematical

Skills:

General mathematic skills.

Mental Capabilities: Ability to concentrate for long periods, follow supervisor instructions, and apply critical thinking to assess and intervene within scope of practice. Maintain patient confidentiality and handle complex health problems with sensitivity.

Interpersonal: Able to work well with others as a team player. Organized, detail oriented, and able to multi-task while managing a heavy load of complex patients. Maintain a neat and professional appearance.

Performance: Able to handle conflicts with patients and staff. Follow established recommendations and guidelines. Meet deadlines as needed.
