Kenniscentrum Sociaal Domein zoekt een Consulent Maatschappelijke Ondersteuning & Regie. Je benadert inwoners actief, onderzoekt hun hulpvraag en regelt passende ondersteuning in het sociaal domein.
Je voert huisbezoeken en gesprekken, coördineert zorg met ketenpartners en regie bij complexe situaties. Ervaring met Meldcode, ZRM en Zorg Ned is vereist en er wordt gewerkt volgens de gemeente CAO.
