Position: UNM Sandoval Regional Medical Center – Department:
Case Management FTE: 1.00 Full Time –
Shift: Days Hourly Compensation: $22.49 – $33.74
Coordinate and provide services and resources to individuals/families to promote rehabilitation, recovery, and resiliency. Identify strengths and barriers to independence and support crisis situations with personalized interventions. Work with a diverse population including children, adults, and geriatric clients.
Detailed Responsibilities- Adhere to Patient Centered Medical Home or Specialty Practice expectations.
- Eliminate barriers to culturally competent medical services and assist patients in accessing community resources.
- Collaborate with medical providers and multidisciplinary teams.
- Facilitate and encourage life skills development (activities of daily living, interpersonal coping, socialization, community functioning).
- Plan for professional growth and acquire knowledge to meet goals.
- Offer information and resources for mental health conditions and relapse prevention.
- Provide education on medication, side effects and adherence.
- Assist in developing individualized service, crisis, and recovery plans.
- Coordinate rehabilitation, medical and other services, including financial benefits and housing support.
- Encourage development of natural supports in workplace, school, family, and community.
- Assess personal strengths, needs, and barriers; conduct ongoing assessments.
- Intervene in crisis situations and develop crisis management plans.
- Track client activity and care plan implementation; conduct post‑discharge follow‑up.
- Document client encounters and billing, and maintain comprehensive files.
- Maintain and report program statistics.
- Implement patient safety policies and report concerns.
Education: Bachelor’s Degree (essential) with related discipline optional. One year of directly related experience (essential).
Credentials: CPR for Healthcare/BLS or Professional Rescuer within 30 days;
Valid New Mexico Driver's License; UNM Vehicle Operator’s Permit within 60 days (essential). Nonessential:
Certified Psychosocial Rehabilitation Practitioner, Licensed Alcohol and Drug Abuse Counselor or LASI eligible.
Physical Conditions: Light work: up to 20 pounds occasionally, up to 10 pounds frequently; may require walking or standing for a significant degree; pushing and pulling of arm or leg controls.
Working Conditions: Minor hazards – dust, fumes, noise; tuberculosis testing; random alcohol and substance testing.
