The Center of Access and Linkage to Healthcare Northwest Region is comprised of seven counties and works to promote health and wellness, improve health outcomes, and assure safety net services for all people in New Mexico. Health Promotion work is concentrated on population health, looking at changes impacting groups of people in communities where they live, work, play and age. Health Promotion strengthens collaborative efforts with internal and external partners with data, technical assistance and other resources at the request of communities.
The position is responsible to assure coordination of local community health improvement activities with the Department of Health by assessing community readiness; raising awareness on identified health issues; providing and interpreting data; identifying appropriate local partners; including partners in planning and implementation of regional and state initiatives; and facilitating communication between local, region and state entities.
Why does the job exist?This position is responsible for supporting the NW Region. This includes but is not limited to, Public Health team, Health Promotion team, Health Councils and Tribal Health Councils and communities within the greater Sandoval County area. The role will address public health issues based on regional priorities and implement special projects and/or grants that address health priorities in communities. This may involve direct client health education, group education, community coalition building, group facilitation, contract, outreach, and grant management and/or generally supporting Public Health Offices and community health councils.
The position will also complete and submit invoices for health councils and maintain tracking and record‑keeping.
- Support, collaborate and promote Public Health Office services through Outreach and Engagement.
- Support, collaborate and build healthy relationships with Tribes, Pueblo's and Nations.
- Providing Technical Assistance.
- Stakeholder Collaboration.
- Promote Mobile Unit services, utilization, and synchronization.
- Invoicing and tracking deliverables.
Key partners include Tribes, Pueblos, and Nations, New Mexico demographic population.
Ideal CandidateBachelor's degree in public health, community health, or related field and four (4) years of any combination of experience including working with communities, working on health or social service‑related matters, working with health councils, behavioral health, and/or health care. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling eight (8) years may substitute for the required education and experience.
Associate's Degree in Social Work, Psychology, Guidance and Counseling, Education, Sociology, Criminal Justice, Criminology, Family Studies/Services, Social Sciences, or Human Services and one (1) year of experience as a social work assistant and/or in social or community coordination, working with communities, working on health or social service related matters, social work/case management experience, and/or behavioral health care. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling three (3) years may substitute for the required education and experience.
EmploymentRequirements
Must possess and maintain a valid Driver's License.
Working ConditionsWork is performed in an office setting: late hours and weekends work may be required. Will be exposed to regular periods of video display terminal and keyboard usage and stressful situations. Travel may be required. Incumbent will work under stress and frequent time constraints. Must have own mode of transportation. Incumbent may be required to furnish a personal vehicle for carrying out assignments.
