CNA – LTC​/SNF

Job Summary:

Provides direct patient care within a Long-Term Care or Skilled Nursing Facility setting. Works under the supervision of nursing staff to support residents' daily needs and ensure their well-being.

Responsibilities:

• Assist residents with activities of daily living, including bathing, dressing, and mobility.
• Monitor and report changes in residents’ conditions to nursing staff.
• Maintain a clean, safe, and comfortable environment for residents.
• Follow infection control and safety protocols at all times.
• Document care activities accurately in compliance with facility policies.
• Support medication administration and vital sign monitoring as directed.

Qualifications:

• Valid CNA license or certification required.
• Proven experience in LTC/SNF environments preferred.
• Ability to communicate effectively with residents and team members.
• Demonstrate compassion, patience, and professionalism in care delivery.
• Must complete required COVID-19 vaccinations or have approved exemptions.
• Ability to adhere to facility policies regarding identification and documentation.
