- Complete required documentation related to admissions, transfers, and discharges.
- Coordinate with nursing staff and support personnel to ensure continuity of care.
- Administer medications and treatments as prescribed.
- Review medication records for completeness and accuracy.
- Monitor and evaluate care plans, updating them as needed based on patient needs.
- Assist in developing and implementing clinical policies, procedures, and care programs.
- Provide leadership and guidance to nursing staff.
- Conduct routine rounds to ensure quality care is being delivered according to care plans.
- Maintain professional relationships with patients, families, healthcare providers, and team members.
- Participate in regulatory surveys and compliance activities as required.
- Document nursing observations, interventions, and patient responses accurately and timely.
- Graduate of an accredited Practical Nursing program.
- Current, active, unrestricted Licensed Practical Nurse (LPN) license.
- Knowledge of nursing principles, practices, and applicable regulations.
- Ability to accurately document clinical information.
- Experience using electronic medical records (EMR) and computerized documentation systems.
- Strong communication, organization, and leadership skills.
