Job Coach
Ogden Area - Riverdale, UT. Schedule is part time (25 hours a week) with a mixture of days/swings and potential weekend/holiday commitments.
Hourly Wage: $15 with potential wage increase upon certification as needed (ACRE Certification highly recommended but not required).
Chrysalis provides a variety of support services for at-risk adults and/or juveniles with intellectual disabilities, who may have medical issues. This position requires the ability to work in stress-filled and safety-sensitive environments, make crucial care decisions, and communicate those decisions effectively to the individual’s team. Under the supervision of the Employment Specialist, the Job Coach will facilitate Vocational Rehabilitation supported and customized employment services, including training individuals on essential job duties, establishing relationships with employers and coworkers, supervising individuals on the job, and other functions within Chrysalis policies to ensure high-quality services.
Essential Duties and Responsibilities- Understand the individual’s essential job responsibilities and implement coaching techniques to train and promote independence in the workplace.
- Identify an individual’s skills, abilities, conditions, and contributions through observation, interviews, and interactions to customize the employer relationship.
- Facilitate natural supports by developing professional relationships with the individual’s employer and coworkers.
- Communicate effectively with the individual’s support team, employer, and coworkers.
- Identify necessary accommodations for workplace success.
- Support problem solving between the employer and the individual; reassess performance and renegotiate as needed with the employer.
- Teach daily living skills as they relate to essential job responsibilities.
- Complete all necessary documentation that clearly reflects the individual, as required by Chrysalis and the State, within established time frames.
- Supervise the individual according to established Support Guidelines.
- Support Person-Centered Support Plans (PCSP) and goals, including applying techniques from Behavior Support Plans.
- Identify and create opportunities to increase quality of life in the workplace.
- Punctuality and regular/consistent attendance at the worksite are required. The job coach must ensure proper oversight of individuals and their worksite schedules to facilitate successful employment outcomes.
- Attend organizational training and provide documentation of certifications required for employment with Chrysalis.
- Understand qualification process for Paratransit or other transportation options, and train individuals on using these services.
- Perform other related duties or projects as assigned by the Employment Specialist, including shifts in Chrysalis homes or day programs as needed.
- Minimum of at least 21 years old
- Pass a criminal background check
- Ability to speak, read and write effectively in English
- Be eligible to drive according to Chrysalis policy
To drive your personal vehicle: either 20-21 years old with no moving violations, or 22 years or older with up to 2 minor moving violations. To drive a company vehicle: at least 21 years old with no moving violations, or 22 years or older with up to 2 minor moving violations. Driving record will be run before interview. Please bring a valid driver’s license to the interview.
Trainingand Certifications
- Complete Workplace Supports Training
- Complete annual CPR, First Aid, OSHA and SOAR training
- Complete annual Human Rights, Abuse Neglect, Maltreatment and Exploitation Training
- Obtain and maintain Food Handler's Permit
- Complete any other required training assigned by Chrysalis
- The program requires flexibility with scheduling and weekend availability. Job Coaches are scheduled based on the individual’s work schedules and may change based on employer needs. Changes are typically communicated in advance to allow planning. Additional hours may be available as the program grows.
- Part-time with potential for full-time. Initially around 20 hours, with potential growth to 30-40 hours including shifts in Chrysalis homes/day programs, based on availability and program needs.
- Understand policies and follow the Provider Code of Conduct/Abuse Neglect, Exploitation, Maltreatment Policy according to state standards.
- Excellent public relations skills when working with individuals and employment personnel.
- Ability to work successfully unsupervised.
- Display integrity, trust, and follow-through in all interactions.
- Follow established safety practices to ensure a safe and healthy work environment.
- Work effectively within a team to enhance service delivery while respecting privacy and HIPAA of the individual.
