Position: COMMUNITY HEALTH WORKER/PROMOTORE I/II

Community Health Worker I/II

Riverside University Health System (RUHS) invites qualified candidates to apply for the position of Community Health Worker I/II with multiple programs throughout the County of Riverside. Pay Scale:
Community Health Worker I: $19.63‑$26.16 Community Health Worker II: $20.71‑$27.60

Responsibilities
  • Build rapport with clients, gather and assess accurate information to identify efficient intervention opportunities.
  • Provide health education, instruction, and information in a manner that clients can understand and address barriers to physical and mental health services.
  • Assist clients in navigating the healthcare system and public health programs by phone and in person.
  • Engage with community members to raise awareness about violence prevention strategies.
  • Build trusting relationships with individuals and families affected by asthma, offering guidance, emotional support, and practical assistance.
  • Connect program participants to community resources, address healthcare barriers and identify health‑related social needs, and make closed‑loop referrals to local human service systems.
  • Provide care management services for clients at highest risk of poor health outcomes.
  • Provide transitional care services (TCS) for clients transferring from one setting or level of care to another and maintain close contact with clients and caretakers.
  • Support clients’ engagement with their primary care physician (PCP), identify and connect clients to services that address SDOH needs, promote wellness and prevention, help members manage chronic diseases, and support efforts to improve maternal and child health.
  • Participate in team collaborations and continuous quality improvement initiatives and projects.
  • Ensure accurate, useful documentation in electronic health records and other tracking systems, in compliance with regulatory requirements and accreditation standards.
Physical Requirements

Ability to drive for extended periods, sit and stand for extended periods, walk, climb stairs, move between field sites, and lift objects weighing up to 25 pounds with or without reasonable accommodation.

Qualifications (Community Health Worker I)

Education: High school graduation, certificate of proficiency issued by the California State Board of Education, or satisfactory G.E.D. score.

Experience: One year of lived experience working with diverse populations such as women with infants, homelessness, substance abuse, justice system involvement, HIV/AIDS patients, older adults, and underserved populations, providing personal first‑hand knowledge of community needs.

Knowledge: Client population and community resources; techniques for group and individual interviewing and counseling; basic record keeping principles and practices; computer equipment and applications.

Ability: Address client populations’ unique needs in a culturally sensitive manner; communicate effectively; understand organization and operation of the department; maintain relationships with community agencies; record accurate case documentation.

Other Requirements

• Valid California Driver’s License may be required.

• Completion of a Department of Health Care Access and Information approved CHW/Promotore training program and possession of a CHW/P/R certificate within 12 months of hire.

• Fluency in a second language may be required, as determined by client population served.

Qualifications (Community Health Worker II)

Education: Same as I.

Experience: One year as a Community Health Worker/Promotore I or equivalent non‑County classification, OR two years of lived experience equivalent to a CHW/Promotore I working with diverse populations.

Knowledge & Ability: Same as I.
