We provide treatment for traumatic brain injuries, spinal cord injuries, CVAs, and other neuro or complex orthopaedic diagnoses in the most functional setting: a patient’s own home and community. We seek Physical Therapist Assistants who want to provide truly patient-centered care in Riverside, CA, and surrounding areas, with PRN positions available.

Our interdisciplinary teams create integrated treatment plans practiced in patients’ homes and communities. Therapists are creative and incorporate a variety of activities to ignite interest while addressing functional goals, resulting in durable outcomes.

Collage Home and Community Rehabilitation Partners creates new paths forward after neurological injury. We provide intensive, individualized services one-on-one with patients, building skills in their own homes and communities. Patients are accepted anywhere on the continuum from severe brain injury to job readiness. Our experienced teams step up to the challenge of building tailored programs for each patient.

• Evaluate and treat impairments, including lower extremity function, mobility, vestibular skills, adaptive equipment needs, and environmental safety.

• Associate Degree in Physical Therapy Assisting from an accredited program required;
  Bachelor’s or Master’s in the same discipline preferred.
• California state certification.
• Knowledge of physical therapy principles, standards, practices, equipment use, and OSHA and infection control regulations.
• Strong preference for experience treating neurologically related physical disorders.
• Excellent planning, organizational, writing, and time‑management skills.
• Strong communication skills with families, professionals, and co‑workers.
• Valid driver’s license.
