Hours:

MON‑FRI 7:30 AM – 5:00 PM. Evenings and weekends may be required.
Spanish Bilingual: level 1 required.

Location:

Riverside City and Indio City.

We are seeking dedicated applicants—especially those who are bilingual and have experience supporting individuals, children, youth, and families with complex medical conditions, behavioral health, and other health barriers. Health Equity Outreach Program is designed to connect individuals with the healthcare and community resources they need to achieve optimal health outcomes. The program builds trust with clients, provides culturally sensitive health education, and helps navigate medical and public health services. Through community outreach, education, and individualized support, participants are linked to essential services addressing social determinants of health—such as housing, food, transportation, and childcare—while promoting wellness, chronic disease management, and maternal and child health.

Compensation:

• Community Health Worker I: $19.63 – $26.16 per hour.
• Community Health Worker II: $20.71 – $27.60 per hour.

Physical Requirement: Ability to drive for extended periods, sit and stand for extended periods, walk, climb stairs, and move between field sites, and lift objects weighing 25–50 pounds with or without reasonable accommodation.

Key Responsibilities:

• Build rapport with clients and gather accurate information to identify efficient and effective opportunities for intervention.
• Provide health education and instruction in a manner that clients can easily understand and address barriers to services.
• Assist clients in navigating the healthcare system and public health programs by phone and in person.
• Engage with community members to raise awareness about violence prevention strategies.
• Build trusting relationships with individuals and families affected by asthma, offering guidance, emotional support, and practical assistance.
• Connect program participants to community resources necessary to achieve healthy outcomes, including housing, food, personal safety, transportation, childcare, and energy assistance.
• Provide care management services for clients at highest risk of poor health outcomes by helping them access clinical and community services.
• Provide transitional care services (TCS) for clients transferring between settings and maintain close contact with clients and caretakers.
• Support clients’ engagement with their primary care physician, identify and connect clients to services that address social determinants of health, promote wellness and prevention, and support chronic disease management.
• Participate in all team collaborations and continuous quality improvement initiatives.
• Ensure documentation in electronic health records and other tracking systems is accurate, useful, and compliant with regulatory requirements.

Community Health Worker I Recruiting Guidelines:

• Education: High‑school graduation, certificate of proficiency issued by the California State Board of Education, or satisfactory G.E.D. score.
• Experience: One year of lived experience working with diverse populations in the community (e.g., women with infants and young children, homelessness, substance abuse, justice‑involved persons, AIDS/HIV patients, older adults, and underserved populations).
