The RoleAs a Nurse Practitioner you will provide exceptional one‑on‑one patient care, support complex patients, educate, create care plans, collaborate with integrated care teams, and coordinate services across providers.
Responsibilities include:
- Support highly complex patients navigating a challenging health system.
- Provide patient education, create dedicated care plans, and give clinical guidance to an integrated care team.
- Work closely with client Integrated Care team for our dialysis patient population.
- Function autonomously and collaboratively to coordinate high‑quality, cost‑effective care.
- Coordinate care among patients, families, physicians, and other team members.
- Administer various tests and screenings.
- Perform comprehensive health assessments and health risk assessments with accurate clinical coding and documentation.
- Maintain strict confidentiality in line with HIPAA regulations.
This full‑time opportunity includes in‑clinic shifts at dialysis centers, nephrology practices, in‑home visits, occasional telehealth appointments, and flexible coverage across regions.
Experience and Qualifications- Current California Nurse Practitioner license.
- Board certified NP in good standing.
- NP certification from ANCC or AANP.
- 2+ years clinical experience in primary care, family medicine, internal medicine, gerontology, or palliative care; home care experience is a plus.
- Working knowledge of chronic care management and medication options.
- Current DEA licensure / prescriptive authority post‑hire.
- Active CPR/BLS certification.
- Knowledge and experience with HCCs.
- Current California driver’s license.
