Primary Care Provider, Nurse Practitioner / Physician Assistant – Full‑Time

• Provide primary care for elderly Medicare/Medi‑Cal participants, including preventive, acute, and chronic disease management.
• Prescribe medications, complete DEA registration, and maintain prescriptive authority.
• Coordinate comprehensive, holistic care with interdisciplinary team members, including home visits, and participate in rotating on‑call schedule as needed.
• Maintain accurate, compliant electronic health records and documentation per program and regulatory standards.

• Licensed Nurse Practitioner or Physician Assistant with at least two years of experience working with the elderly.
• Prescriptive authority, DEA registration, and eligibility to serve Medicare/Medi‑Cal participants.
• Strong communication skills, ability to build trust with seniors and families, and commitment to a teambased model of care.
• Valid California professional license and current registration with relevant certifying agencies.

• Compensation range: $145,399 – $174,478 per year + quarterly bonus + equity grants.
