The Welbe Health PACE program helps seniors stay in their homes and communities by providing comprehensive medical care and community-based services. The Home Health RN plays a vital role in achieving our mission by performing skilled duties at home under CMS approval, bringing convenient and necessary care to our participants’ homes.

• Conduct Home Health Nursing Assessments to determine equipment needs and actively participate in IDT meetings to develop care plans
• Deliver and document nursing interventions as agreed upon in the care plans, including maintaining a safe environment, responding to physician orders, administering medications, and performing ordered tests and treatments
• Instruct participants, families, and caregivers regarding disease process, self‑care techniques, and prevention strategies
• Track and monitor Home Health scheduling in collaboration with the Home Health Manager or scheduling contact
• Collaborate with the Welbe Health Physician and IDT members to assist in smooth care transitions between settings

• Graduate of an Accredited School of Nursing
• Unencumbered RN License
• Minimum of one year of nursing experience in a clinical setting with a frail or elderly population; home health agency experience preferred
• Reliable means of transportation

Base range: $47.74 – $63.02 per hour, plus bonus and equity. Pay will be adjusted based on experience and other qualifications.
