Job OverviewWe are seeking an energetic Licensed Vocational Nurse (LVN) to join our skilled professional team. The role involves traveling to patients’ homes throughout the day and documenting care via electronic device provided by Lorian Health. All documentation will be done through a smartphone. The LVN will deliver direct nursing care as prescribed by the physician, under the direction and supervision of the Registered Nurse (RN), in compliance with the State LVN Practice Act and Lorian Health policies.
Responsibilities- Travel to patients’ homes and provide nursing care as prescribed by the physician.
- Observe patient/client regularly for changes in condition and discuss changes with the patient, family or caregiver.
- Document and report changes as necessary to the attending physician.
- Educate and instruct the patient, family and caregivers regarding the patient’s care.
- Perform other job‑related tasks as requested.
- LVN/LPN licensed to practice in the state of practice and in good standing.
- Graduate of an approved school of practical/vocational nursing.
- One (1) year experience as a professional nurse in an acute setting.
- Current CPR certification (BLS required).
- Valid driver's license and insured automobile in accordance with LORIAN requirements.
- Ability to read and follow written instructions and document care in the patient’s electronic medical record.
- Excellent observation, good nursing judgment and strong oral and written communication skills.
- Self‑directing with the ability to work with little direct supervision.
- Ability to work with adults (≥18 years) demonstrating tact and patience.
- Flexible and cooperative in fulfilling role obligations.
- Participates in in‑service education programs and clinical meetings.
- Adheres to infection‑control policies and protocols.
- Maintains confidential patient records in accordance with HIPAA and applicable state laws.

Pay‑Per‑Visit model rates range from $30/visit to $40/visit, commensurate with experience and type of visit. The estimated yearly compensation is reflected in this posting.
