MDS Coordinator​/LVN

Position Overview: Under the supervision of the Director of Nursing Services, the Medicare/MDS Coordinator is responsible for notifying and coordinating the Interdisciplinary Team (IDT) for MDS assessment completion in accordance with State and Federal regulations.

• Coordinate the Medicare/MDS resident assessment process.
• Ensure timely completion of the MDS Assessment by the Interdisciplinary Team.
• Develop, implement, and evaluate the plan of care.
• Coordinate and perform treatments, medications, or nursing interventions as indicated by the resident's care plan or physician's orders.
• Provide nursing care in accordance with infection control standards.
• Follow safety policies during nursing care.
• Initiate emergency measures as needed according to center policy and nursing standards.

Requirements:

• Current LVN License in California.
• At least 2 years of experience as an MDS Coordinator in a Skilled Nursing Facility.
• Background check with no history of abuse.
