Mosaic Life Care St. Joseph MO & St. Joseph and St. Luke's North - North Kansas City, MO.
Schedule:
Part time – 5 hours/day;
Monday–Friday, 8 am to 5 pm; no holidays or weekends.
The Dialysis Coordinator is responsible for the development and implementation of DaVita’s Patient Pathways Program, including:
- Facilitating patient education on outpatient dialysis, modality options, vascular access, insurance, and cost.
- Processing timely dialysis place referrals and building relationships with local hospitals, doctors, and other kidney care providers.
- Improving the patient education process by analyzing and sharing best practices.
- Communicating with and treating patients with empathy, meeting individually to provide educational materials.
- Identifying, resolving, or escalating patient issues and barriers to placement.
- Working with hospital case managers, social workers, and physicians to place patients in outpatient dialysis clinics.
- Assisting with patient transitions from hospitals to dialysis facilities and ensuring satisfaction.
- Documenting referrals and recording patient data in the electronic tracking system.
- Conducting operating reviews, reporting monthly, and adhering to audit, compliance, and reporting requirements.
- Developing and implementing new ways for Patient Pathways to differentiate itself; participating in special projects.
- Associate degree required;
Bachelor’s in social work, nursing or related field strongly preferred. - Knowledge of health systems, community resources, medical terminology, and the ability to read medical records.
- Proficiency in electronic medical records and related technology.
- Strong communication, organizational, and time‑management skills; ability to handle multiple projects.
- At least one year of discharge planning and/or case management support role, or experience with dialysis patients.
- At least one year of experience working with physicians.
- Current case‑management/social‑work certification or willingness to obtain it.
- Valid driver’s license and ability to travel locally.
- Strong work ethic and desire to serve patients.
