Continuum Rehab Group is seeking a PRN Weekend Physical Therapist to provide high‑quality care in our in‑house therapy program across multiple Midwest locations.

Base pay range: $50.00/hr - $58.00/hr

Continuum Rehab Group provides in‑house therapy with skilled nursing facilities in Texas, New Mexico, Arizona, Indiana, and Missouri. This role offers $50-$58 DOE. Experience &

Qualifications:

Must have graduated from an accredited university and hold a valid (or in‑process) license in the appropriate state. This position is open to therapists at all experience levels.

Some Duties for This Position Include:

• Utilizes evidence‑based practice to support clinical interventions.
• Provides rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders per regulatory and clinical practice requirements.
• Provides skilled therapy services to patients who need rehabilitation services.
• Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
• Supervises physical therapy assistants, temporary licensees, students, and support personnel in accordance with state licensure requirements and professional standards.
