Patient Care Coordinator

Description We are looking for a Patient Care Coordinator for a Contract position based in Riverside, Missouri. This role supports the referral and intake process by working closely with physicians, care teams, and patients to help arrange timely services and confirm coverage details. The ideal candidate brings strong judgment, attention to detail, and a patient-centered approach while coordinating documentation, scheduling, and communication across multiple departments.

Responsibilities:

- Manage incoming referrals from physicians, hospitals, care facilities, and community partners, ensuring each case is processed accurately and without delay.

- Gather and review required clinical and insurance documentation, including service initiation paperwork, to support admission and care coordination activities.

- Partner with intake, nursing, therapy, pharmacy, and administrative teams to organize smooth onboarding for new patients and align service delivery needs.

- Confirm benefit eligibility, secure prior authorizations when needed, and verify approval before services begin.

- Review patient financial and coverage documents with care and accuracy so that responsibilities and benefits are clearly documented.

- Maintain active patient orders and authorizations by sending renewal requests to physicians and tracking responses for continued service.

- Coordinate schedules, equipment or supply availability, and delivery-related documentation to meet physician orders and patient needs.

- Address patient questions and resolve coordination issues by providing clear updates and working with internal teams to remove barriers to care.

- Communicate professionally with clinicians, field staff, and referral sources while supporting timely, objective, and well-coordinated information sharing. Requirements - Experience coordinating patient referrals, scheduling, or intake activities in a healthcare setting.

- Working knowledge of medical insurance verification, authorization processes, and coverage documentation.

- Ability to manage patient medical records and maintain accurate, organized documentation.

- Familiarity with scheduling appointments and coordinating services based on physician orders.

- Experience assisting patients and families with questions in a detail-oriented and compassionate manner.

- Understanding of durable medical equipment coordination or related healthcare service support.

- Strong communication, problem-solving, and time management skills.

- Proficiency with common software applications used for healthcare coordination and record management. Talent Match®
