Nurse Practitioner

Overview

Nurse Practitioner position at Liberty Health focused on hospice and palliative care. The role is per diem covering Halifax and surrounding counties.

• Implement palliative care program requirements.
• Complete medical visits with patients per guidelines and coordinate care with the Palliative Care team as clinically indicated.
• Conduct face-to-face encounter to provide re-certification information to Medical Director/Associate Medical Director for hospice patients.
• Provide clinical findings from the face-to-face encounter to the Primary Care Physician and/or supervising Medical Director/Associate Medical Director.
• Communicate with patients/families at all levels.
• Communicate with all agency personnel.

• Master’s degree in Nursing required, with current licensure in the state of practice.
• Certification as an Adult or Family Nurse Practitioner.
• Experience in Hospice or Palliative Care nursing preferred.
• Knowledge of the framework, function, and organization of a hospice care program.
• Excellent communication and organization skills.
