Education, Experience, Certification and Licensure Requirements
  • Graduate of an accredited school of nursing with a diploma or degree.
  • Licensed as a Licensed Practical Nurse in Virginia.
  • CPR/AED certification required.
Job Responsibilities
  • Assist team members with resident services and activities of daily living as necessary to ensure quality services. Make shift rounds with physicians. Follow proper infection control practices. Assess resident injuries and render first aid or refer resident for physician and/or emergency room treatment; complete incident reports. Respond to resident call light requests promptly. May respond to emergency calls in Independent Living.
  • Receive the report from the off-going shift and give the report to the oncoming shift. Communicate about resident condition with other team members; communicate changes in resident condition to the physician, guardian, and family. Notify clinical leader of major problems and recurrent grievances. Keep dining team informed of diet changes.
  • Within licensure, perform resident assessments upon admission; complete documentation for assessments, treatments, reactions, and outcomes; notify appropriate resources (e.g., dietitian, physical therapist, etc.) of the need for evaluation; initiate the development of care plans (and MDS in Healthcare) for household/neighborhood residents; evaluate resident response to plan of care and recommend additional modifications as indicated.
  • Administer medications and treatments as prescribed by physician and nursing orders, according to regulations and established procedures.
  • Perform clinical rounds to ensure resident safety, comfort, and regulatory compliance; evaluate resident physical and emotional status. Educate residents (and others) about medical issues and healthy lifestyle choices.
  • Maintain resident clinical records according to established policies and procedures.
  • Ensure the clinical operations in their designated area, household or neighborhood comply with federal, state, and local regulations and the Nurse Practice Act.
  • Attend in-service education programs, team conferences, and nursing services conferences as scheduled; complete in-service programs as assigned.
  • Train other clinical team members in nursing skills, and all team members in best practices. Serve as a role model of professional nursing practices. Advocate for quality clinical services.
  • All other duties as assigned.
Qualifications
  • Conceptual knowledge of theories, practices, and procedures within medication and nursing assistance, typically acquired through a nursing degree.
  • Verbal and written communication skills required.
  • Demonstrates working knowledge of common medical processes and treatments.
Physical Requirements and Working Conditions
  • May include frequent lifting (up to 50lbs).
  • Extensive walking, bending, stooping, standing.
  • Ability to work in a fast‑paced environment with multiple tasks.
  • Ability to work inside and outside to effectively make rounds daily throughout the facility and to respond to problems and emergencies.
