Overview
Our busy nephrology practice is seeking a dedicated Nurse Practitioner to join our team and work collaboratively with our providers to deliver high-quality, patient-centered care.
Responsibilities- Manage and treat patients with Chronic Kidney Disease (CKD) and End-Stage Renal Disease (ESRD)
- Perform comprehensive patient assessments and physical examinations
- Develop, implement, and monitor individualized treatment plans
- Order and interpret diagnostic tests and laboratory studies
- Provide patient and family education regarding kidney disease, treatment options, and lifestyle modifications
- Collaborate with physicians and other healthcare professionals to ensure continuity of care
- Maintain accurate and timely medical documentation
- Participate in ongoing quality improvement and patient care initiatives
- Current Nurse Practitioner licensure (state-specific)
- Experience in nephrology or chronic disease management preferred
- Strong clinical, communication, and organizational skills
- Ability to work effectively in a fast-paced clinical environment
- Commitment to providing compassionate, evidence-based care
