Case Manager Registered Nurse - Field in Roanoke VA

• Pay: $60,522 - $129,615/year
• Location: Virginia, VA
• Schedule: Full-time, Monday–Friday, 8:00 AM–5:00 PM
• Travel: Up to 75% travel required

• Conduct in-depth, face-to-face assessments to identify member needs
• Develop and implement individualized care plans
• Coordinate medical, behavioral, and community-based services
• Monitor progress and adjust care strategies to improve outcomes
• Advocate for members to ensure access to high-quality, appropriate care

Education:

• Associate's Degree or Bachelor's in Nursing required;
  Bachelor's Degree in Nursing preferred

Experience:

• 3 years Clinical Experience (preferably in Medical-Surgical Nursing, Behavioral Health, Long-term Care)
• 1+ years Managed Care experience
