Position SummaryThe Patient Care RN is responsible for providing coordinated care to patients by developing, monitoring, and evaluating interdisciplinary care to ensure improved patient outcomes. Develops interdisciplinary care plan and other case management tools by participating in meetings; coordinating information and care requirements with other care providers; resolving issues that could affect smooth care progression; fostering peer support; providing education to others regarding the case management process.
GeneralDuties and Responsibilities
- Initiates care plan assessment with patients and families by explaining the role of the patient care RN; provides educational information; works collaboratively with providers on related treatments, procedures, medications and continuing care requirements.
- Evaluates patients by acute and risk including management of patients with multiple co-morbidities or high risk for readmission to a hospital setting; maintains a registry to interface with patients and provide referrals to other supportive services as appropriate.
- Coordinates and monitors delivery of care following hospital admission, discharge and ER visits with internal and external healthcare providers to include telephonic patient rounding; provides support of self-management of disease and behavior modifications; documents and reviews care plans with patients and direct care providers.
- Interacts with necessary facilities to coordinate and expedite scheduling and completion of tests, procedures and consults to ensure care requirements are completed in a timely manner.
- Participates in patient-centered team meetings with interdisciplinary departments to provide care progression updates; resolves challenges; identifies necessary resources for effective case management and establishes ongoing communication plans.
- Participates on evaluation teams for data collection, health outcomes reporting, clinical audits and program evaluation related to the Patient-Centered Medical Home and Medical Neighborhood initiatives.
- Follows medical records documentation guidelines regarding timeliness of reviews and completeness of documentation; obtains and documents accurate and thorough patient medical history including pertinent history, pain assessment, current medications, identified health risk factors and other needed information.
- Maintains patient care database and reports; provides updates on trends and analysis to management teams; compiles and reports necessary metrics as directed for regulatory compliance.
- Evaluates clinical care program, utilization of resources and oversees development of new clinical tools, forms and procedures.
- Complies with all established safety procedures to ensure a safe environment for patients, visitors and staff.
- Participates in performance improvement activities.
- Performs other duties as assigned.
- Current IL RN License Required
- BLS Required
- Registered Nurse minimum 3 years Preferred
- Clinical or Patient Case Management minimum 1 year Preferred
- Strong computer skills including Microsoft Excel and Outlook
- Ability to work independently, prioritize and complete tasks in a timely manner
- Strong interpersonal, organizational and time management skills
- Excellent written and verbal communication skills, be professional, polished and articulate well
