About the Role

Complex Care Assessor – Layola Care (Full-Time) — Based in Rochdale

Layola Care are seeking a skilled and compassionate Complex Care Assessor to join our professional, friendly team. This is a vital role supporting individuals with complex physical, behavioural, and mental health needs.

Responsibilities

Complete comprehensive care assessments and reviews
Develop and update individualised care plans
Liaise closely with families, health professionals, and care teams
Support staff with guidance, training, and supervision
Ensure compliance with Care Quality Commission (CQC) standards and Layola Care policies
Take part in the shared on-call rota

This is office hours 9am-5pm but you must take part in the on‑call rota.

About You

Experienced in health or social care (Senior Carer, Nurse, Team Leader, Care Coordinator)
Confident assessing complex needs and developing detailed care plans
Organised, proactive, and an excellent communicator
Compassionate and committed to person-centred, high-quality care
A driver with access to a vehicle (essential)
