About the RoleAs a Support Worker, you will provide person-centred care to individuals in their own homes, supporting them with a variety of health needs, including:
- Learning disabilities
- Mental health conditions
- Physical disabilities
- Personal care needs
- Complex care requirements
You will play a vital role in ensuring that those in your care live fulfilling, independent, and happy lives. Your responsibilities will include:
- Assisting with personal care and hygiene routines
- Administering and managing medication in strict compliance with care plans
- Supporting individuals to access the community, engage in social activities, and develop healthy relationships
- Encouraging independence in daily tasks such as meal preparation, household chores, and financial management
- Maintaining a clean, safe, and accessible home environment
- Supporting with day-to-day activities, including attending appointments, shopping, and engaging in hobbies
- Assisting individuals in managing emotions and behaviours, promoting positive mental well-being
- Providing companionship and emotional support
- Supporting individuals in external settings such as hospitals, holidays, or police stations if required
- Keeping accurate records and reporting to the Service Manager
We offer flexible shifts to suit your lifestyle, including:
- Days or nights available (We offer outreach support - 3hours – 10 hours per day and full days (12 hours).
- Sleep-in shifts available also.
Roles available in Rochdale.
This is a variable hour contract. Please include your availability for work in your application.
