We are looking to welcome an experienced, qualified General Practice Nurse into our practice.
You must be registered with the NMC and have experience of dealing with long term condition management and QOF.
This post is 24-30 hours a week. Salary will depend on qualifications and experience.
Main duties of the job- Diabetes
- Asthma
- COPD
- CHD
- Requesting pathology tests and processing the results, advising patients accordingly
- Immunisations (routine, childhood and travel)
- Understand the importance of evidence based practice
- Broad knowledge of clinical governance
- Ability to record accurate clinical notes
- Ability to work within own scope of practice and understand when to refer to GPs
- Understanding of infection prevention and control measures
- Knowledge of health promotion strategies
- Understand the requirement for PGDs and associated policy
We are a small practice with 6,100 patients,

Job responsibilitiesThe following are the core responsibilities of the practice nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels
- To implement and evaluate individual treatment plans for chronic disease patients that promote health and well-being
- Practice independently in accordance with NMC code of conduct and with reference to practice guidelines
- Ensure continuation of own personal and professional development, facilitated by utilisation of clinical supervision networks.
- Provide focus for nursing expertise within practice, advising staff, appropriate to level of competence
- Administer adult and child immunisations and vaccinations in accordance with national and local programmes. Is competent in anaphylaxis and resuscitation techniques. Ensure safe storage, rotation and disposal of vaccines and drugs and adequate stock levels are maintained.
- Takes a lead in areas such as Asthma and COPD management, undertakes the running of disease management clinics and provides specialist nursing care for the patients or communities that the practice serves.
- Management of chronic diseases which includes asthma, hypertension, COPD, CVA/TIA, CHD, and CHD risk assessment and management. Initiates the process of diagnosis with patients suspected to have chronic disease
- Initiates and carries our programmes of Health promotion and screening, cervical and sexual health screening.
- Lead for Infection Control
- Qualified Nurse
- Registered with NMC
- Experience of working in a GP practice
- Post graduate diploma or degree (Chronic Disease Management)
