Helix is an innovative, fast-paced human service agency in Minnesota working to eliminate barriers in accessing mental health, chemical health, and housing services for people throughout Minnesota that are under-served. Helix provides five (5) areas of service across the state of Minnesota:
Community-Based Mental

Health Services, Low Intensity Outpatient Treatment Services and SBIRT, Mental Health Targeted Case Management (MH-TCM), Community Health Worker (CHW) Services, and Housing Support. Helix provides culturally responsive care that is individualized to meet the needs of people served.

POSITION SUMMARY:

This position will provide individual adult mental health case management and supportive housing services to support clients with serious and persistent mental health diagnoses. Helix serves individuals in the community who have historically struggled to access services to include individuals who are experiencing homelessness/housing instability, justice-involved, and working to recover/manage substance use/ mental health symptoms. Activities may include crisis intervention,

case management, client advocacy, prevention, and education. Helix's services aim to increase psychiatric stability, promote social competencies, facilitate personal and emotional stability, and maintain independent living in the community.

ESSENTIAL DUTIES & RESPONSIBILITIES:

Provide housing focused community-based mental health case management services to qualifying adults, ages 18 and older, diagnosed with a serious and persistent mental illness.

Assist individuals with locating, securing, moving into and maintaining housing as part of the Steps of Strategy, Nivon and The Housing Consultants partnership.

As identified, support other Helix MH-TCMs will locating and securing housing opportunities.

Complete a minimum of 30 billable mental health encounters a month.

Complete Functional Assessments, Individual Community Support Plans, and other documentation required for Rule 79 case management.

Complete all required documentation per Minnesota statute and case notes within 48 hours of the encounter.

Engage with participants from an empowerment, strength-based perspective and promote participant self-determination and develop trusting, therapeutic relationships with participants.

Support participants in individual sessions to assist them in meeting goals around substance use, mental health, physical health, employment, transportation, housing, financial, relationships, and/or leisure time.

Refer participants to community resources, including housing and treatment programs, to assist in recovery from mental or physical illness, following through to ensure service efficacy.

Develop, monitor, evaluate and modify treatment plans according to changes in participant status from a person-centered perspective. Document participant progress. Ensure that treatment plans have a comprehensive housing goal that focuses on housing transition and stability.

Assist participants in adhering to treatment plans, such as setting up appointments, arranging for transportation to appointments, or providing support.

Collaborate with counselors, physicians, nurses, social service providers and other professionals to plan or coordinate treatment, drawing on social work experience and needs.

Coordinate care with others who provide services to participants. Include family members and natural supports in the care of the participants, as applicable, and help assist them in understanding, dealing with, or supporting the participant.

Interview participants, review records, conduct assessments, or confer with other professionals to evaluate the mental or physical condition of participants.

Educate participants or community members about mental or physical illness, abuse, medication, or available community resources.

Increase social services knowledge and best practices by reviewing current literature, conducting social research, or attending seminars, training workshops, or classes.

Attend all agency meetings and trainings.

Receive weekly one-on-one supervision to ensure ongoing support, accountability, and skill development.
