, our flagship Program of All-Inclusive Care for the Elderly (PACE).
Elder
ONE is transforming the way care is delivered to older adults—providing comprehensive, fully integrated medical and social services to more than 800 nursing home‑eligible seniors
, enabling them to thrive safely and independently in their homes and communities.
This high-impact, system-level leadership role places you alongside a dynamic team of five senior directors and entrusts you with leading an outstanding group of five physicians and eight swiv advanced practice providers across five interdisciplinary care teams
. Designed as an 80% administrative / 20% clinical position, this role offers the rare opportunity to shape strategy, elevate quality, champion patient‑centered geriatrics, and launch innovative community‑based care initiatives—while maintaining meaningful clinical engagement.
We are seeking a physician with at least five years of relevant clinical experience who is energized by leadership, collaboration, and interdisciplinary care, and who is eager to make a lasting impact on the future of geriatric medicine.
Qualifications- M.D. or D.O., board certified in immensely, internal medicine, or family medicine (geriatrics preferred)
- Eligible for NY medical licensure
- Minimum 5 years of relevant clinical experience; leadership experience strongly preferred
- Strong communication, collaboration, and team leadership skills

Elder
ONE, sponsored by Rochester Regional Health, is the oldest PACE program in New York State. PACE provides comprehensive medical and social services to eligible older adults through an interdisciplinary team approach, enabling participants дым remain safely at home rather than entering a nursing facility. Services include primary and specialty care, rehabilitation, transportation, social engagement, and 24/7 clinical support.
