Base Pay Range

$90,000 - $110,000 per year

$15,000 sign‑on bonus or student loan assistance.

Our MJHS Medical Associates, P.C. is a group of Nurse Practitioners, Physician Assistants, RN Case Managers and LPNs who provide care to Elderplan members who are residents of assisted living and long‑term care facilities, as well as to those living at home.

An employee in this role utilizes clinical assessments performed by MJHS Med PC providers to implement member’s plan of care. In close collaboration with the assigned provider, the RN case manager develops, implements, and monitors the care plan and ensures continuity of care across all healthcare settings. The RN case manager ensures that an interdisciplinary team (NP/PA, PCP, member/member’s representatives, and specialists) is involved in the care‑planning process.

The case manager utilizes benefit structures and community‑based resources to meet member’s needs and must be on site in ISNP/IE‑SNP partnered facilities.

• RN BSN;
  Master’s degree preferred
• Minimum two years of clinical experience in long‑term care or community settings servicing a geriatric population; managed care or home‑care experience preferred
• Working knowledge of Medicare and Medicaid regulations
• Prior experience in an interdisciplinary service delivery environment preferred
