Job Description:

Title: Community Habilitation Worker

Classification: Full or Part-Time

Pay Rate: $15.00/hr (flexible days, evenings and some weekend hours)

SUMMARY:

Under the direct supervision of the Disability Service Program Manager, the Community Habilitation Worker provides education, instruction, direct services, and support. The Community Habilitation Worker promotes consumer choice and individualized outcomes that empower consumers to achieve their goals and live as optimally as possible. The Community Habilitation Worker and the consumer work together to achieve the goals stated in each individual’s plan.

Job Duties

  • Ensure a person-centered approach is used in all aspects of service delivery.
  • Assist with the development of the consumers’ Community Habilitation Plans.
  • Ensure goals reflected in the consumers’ plans are addressed and worked on with the consumer.
  • Act as an advocate for consumers and encourage self-advocacy and choices.
  • Assist individuals in performing daily living activities with as much independence as possible.
  • Assist in the development of social skills by providing education, training, and role modeling.
  • Assist individuals in community integration and developing relationships.
  • Provide training and education in health-related issues, including specialized dietary needs.
  • Assist individuals with finances and budgeting such as paying bills and shopping.
  • Attend meetings and individual service plan (ISP) reviews for consumers.
  • Complete weekly schedule of appointments and required documentation.
  • Ensure the safety and well-being of consumers at all times.
  • Attend program, training, and staff meetings as scheduled.
  • Transport clients as required by their service plan.
  • Other job duties as assigned.
  • Knowledge, Skill and Abilities

  • Maintain professional relationships with consumers, co-workers, and other agencies.
  • Ability to interact with staff and consumers of diverse cultural, racial, and ethnic backgrounds.
  • Must possess good record-keeping and reporting skills, with excellent attention to detail.
  • Possess effective communication skills, oral and written.
  • Maintain a general working knowledge of community resources.
  • Familiarity with computers and Microsoft applications such as Outlook and Word.
  • Must be willing to work occasional evening and weekend hours.

  • Must be able to physically perform stooping, kneeling, walking, lifting, grasping, pulling, standing, talking, hearing.
  • Must be able to perform light work exerting up to pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.
  • Qualifications


  • Preferred:
    High school diploma or equivalent and one (1) year of experience working in a related field.
  • Requires a valid NYS driver’s license, must have reliable transportation, and pass a driving background check.
  • Must complete Employee Criminal History review (fingerprinting) and training mandated by the NYS Office of Children and Family Services (OCFS).
