40 Hours/week | Starting ASAP for a 3-month duration
550-bed acute care facility
Travel offered, pending location | Acute care experience preferred
Weekdays with monthly weekend rotations

• Conduct nutrition assessment for patients at nutritional risk per facility policies and procedures.
• Develop and implement an individualized plan for nutrition intervention based on nutrition assessment and evidence-based standards.
• Assess educational needs and provide nutrition counseling for individuals and groups.
• Collaborate with the interdisciplinary team in meetings and/or rounds.
• Use electronic charting and the Nutrition Care Process; write PES statements to document the plan of care.
• Work cooperatively with food service staff to ensure conformance to diet prescriptions.
• Demonstrate accountability for the proper use of patients’ protected health information.
• Perform work independently with minimal training.
