Community Health Care Worker – Enlace, formerly WNY Coalition of Farm worker Serving Agencies
Salary: Full-time $36,400 - $43,680 (based on experience)
Reporting: Reports to Program Director
Summary: The Community Health Worker (CHW) plays a critical role in advancing health equity for migrants, farm workers, and other underserved individuals by addressing social care needs and improving access to high-quality, culturally responsive healthcare and community-based services. The CHW builds trusted relationships within the community and serves as a bridge between participants and Enlace’s Social Care Network (SCN), a coordinated system that connects eligible members to essential supports such as housing, food access, transportation, and other services that address social determinants of health.
This role directly supports Enlace’s mission to promote justice, dignity, and equity for migrants and farm workers through advocacy, education, outreach, and direct services. We empower our communities to achieve self-sufficiency by fostering collaboration among partners.
- Establish a visible presence in the community and serve as a trusted resource for health and social service information.
- Conduct outreach and education in community settings, including migrant labor camps, homes, and community events.
- Collaborate with partners to organize and support outreach activities, including intake assistance, interpretation, know your rights and health education.
- Conduct screenings to determine eligibility for the Social Care Network (SCN).
- Conduct Case Management to connect individuals and families to healthcare providers and social service systems.
- Assist clients in identifying priorities, navigating services, and maintaining participation in safety-net programs.
- Support care coordination and track referrals, monitor client progress, and ensure continuity of care through follow-up.
- Advocate with and on behalf of underserved populations to address barriers to care and services. This might include non-profits, governmental agencies, law enforcement among others.
- Provide culturally and linguistically appropriate health education, rights education and informal counseling.
- Partner with community organizations to strengthen collaboration and enhance service delivery.
- Maintain accurate and timely records of client interactions, referrals, and outcomes in accordance with FLIPA, funder requirements, and organizational standards.
- Adhere to HIPAA, FERPA, and all confidentiality requirements when handling client information.
- Document outreach activities and client services using Google Workspace, Salesforce, and Cerenade.
- Assist with preparation of reports, grant documentation, and outreach data as needed.
- Participate in staff meetings, trainings, and outreach initiatives and events.
- Create and update outreach materials, flyers, social media content, and presentations using Google Docs, Slides, Canva, or similar tools.
- Coordinate logistics for outreach activities, including transportation, setup, and materials distribution.
- Collaborate closely with internal teams to ensure alignment with program goals and effective communication.
- Uphold Enlace’s Standards of Conduct, Compliance Program, and organizational policies.
- Perform other duties as assigned.
- Associate’s Degree or equivalent required.
- Experience working with underserved or culturally diverse populations preferred.
- Commitment to immigrant justice, farm worker and migrant rights, and racial equity.
- Valid New York State driver’s license and auto insurance in compliance with Enlace underwriting requirements.
- Clean driving record required for driving on behalf of Enlace.
- Strong interpersonal, communication, and presentation skills.
- Bilingual English/Spanish proficiency (required; proficiency verified).
- Proficiency in Google Workspace and computer-based systems, including CRM platforms.
