Day, Evening – All candidates must be willing to work rotating weekends and rotating holidays.

1 year of elder care or direct care experience preferred but not required.

NYS Nursing Licensure (LPN) required.

$500 Sign‑on bonus available.

• Implement and evaluate the delivery of health services to elders with memory impairment.
• Supervise, provide assistance and oversee medication management and administer routine and PRN medications including diabetic management, insulin injections and blood‑gl refer tests in compliance with nursing standards and NYS regulations.
• Ensure progress notes are informative and descriptive of services provided and of the elders’ response to services.
• Assist in set‑up of medical record upon new admission; medication list, MAR, admission note, service delivery record, etc.
• Assist in providing daily care to elders.
• Function as an advocate for elders, coordinating and linking all aspects of their care with the case manager.칕
• Ensure that Care Partners are aware of each elder’s wellness plan and that these plans are used to provide daily care.
• Make daily roundsРазмер household roles are completed. Assist as needed.
• Investigate complaints and grievances made by elders or families and communicate to leadership.
• Develop and implement effective communication systems between staff; foster positive interpersonal relationships.

Hourly Wage: $25 - $29
