Project Food Box in Rochester, NY is seeking a Lead, Community Outreach and Care Coordination to launch and run our Upstate New York field team. This player-coach role will supervise four Community Outreach Coordinators across 14 counties while performing screening, navigation, and enrollment work in the field.
You’ll own regional partnerships with health systems, FQHCs, food banks, and community organizations, report weekly on activities, and drive enrollment through Find Help and ECM.

The advertised compensation is 65.000 - 75.000.
